Per organization, within a headcount band. Starter covers up to 15 people, Growth up to 50, Professional up to 100. Somebody joining mid-month does not change the bill until you cross a band.
You move to the next plan. We would rather tell you that before you hit the limit than let you find out when somebody cannot be added, so we will flag it as you approach it.

Yes. Employees can be imported, and departments and attendance policies are set up during onboarding rather than left for you to configure from an empty screen.
